In accordance with Texas law, clients have the right to request access to their health care records and to contact state agencies regarding their care.

Requesting Health Care Records

To request a copy of your health care records or a summary of progress, please submit your request to our office in writing. Requests are processed in accordance with applicable state and federal laws. Fees may apply.
